注意事項：

1. 本申請表統一由「組長」填寫，請確認填寫內容正確後再送出，請勿重複提交。
2. 表單送出後即完成申請，恕無法修改，請務必審慎填寫。
3. 本表單僅受理「自主學習計畫」申請，其餘自學計畫請依各相關規定辦理。
4. 申請截止時間為115年9月23日（三）14:00止，逾期將不予受理，亦不接受其他方式補交，敬請見諒。

【申請完成提醒】

• 表單送出後，畫面將顯示「已收到申請」，系統亦會自動發送確認信。
• 後續審核錄取結果，將另以 Email 通知各組全體成員，敬請留意您的信箱。

Important Notes:

1.  This form must be completed by the Team Leader. Please ensure all information is accurate and submit only once.
2.  Once submitted, the application cannot be modified.
3.  This form applies only to the Self-Directed Learning Program. Other applications should follow relevant regulations.
4.  Deadline: September 23, 2026 (Wed) at 14:00. Late or alternative submissions will not be accepted.

• A confirmation message and email will be sent after submission.
• Results will be notified via email. Please check your inbox.
