欲申請114上優良教學助理遴選之同學，請於115年5月31日下午4:00前上傳(需先登入系統)「申請資料」及「佐證資料」。

▲申請資料：請下載「11401-優良教學助理申請資料」填寫並上傳，內容可打字，簽名處須親筆簽名

▲佐證資料：

請將教學助理協助課程之相關資料整理成一完整歷程檔案(檔案格式請為壓縮檔rar或zip)，其內容可包含

(視教學助理實際情形準備，不需每項目皆有) 

• 課業諮詢輔導紀錄及照片
• 為輔導同學課業所準備的講義資料
• 協助教師經營 Moodle 數位教學平臺的網頁資料留存
• 數位教材製作 ( 講義或投影片 )
• 其他有助於審查之相關資料

Students who wish to apply for the Outstanding Teaching Assistant award for the 1st semester of Year 114 must upload the application materials and supporting documents (login to the system required) by 4:00 p.m., May 31, 2026.

▲Application Materials: Please download the “11401-Application Form for Outstanding Teaching Assistants”, complete it (typing is allowed), and upload it. Signatures must be handwritten.

▲Supporting Documents:

Please compile all relevant materials related to the teaching assistant’s support for the course into a complete portfolio file (RAR or ZIP format). The contents may include:

(Prepare according to the actual experience of the teaching assistant; not all items are required.) 

• Academic Consultation Records and Photos
• Handouts Prepared for Student Academic Support
• Moodle Support Materials
• Digital Teaching Materials (Handouts or Slides)
• Other Relevant Supporting Materials
