【學務處衛生保健組】台北校區急救訓練研習報名至5/4截止【Campus Health Services Section】 Taipei Campus First Aid Training Workshop Registration open through May 4

一、活動名稱：急救訓練研習(有證照)。

1. Activity Name: First Aid Training Workshop (with certificate)

二、活動時間：115年6月2日（星期二）12:00-16:00

2. Activity Time: 12:00-16:00, June 2, 2026 (Tuesday)

三、活動地點：B901、B902

3. Activity Venue: B901, B902

四、活動報名請填寫此eForm電子表單，資料須填寫完整且正確，否則證照資料錯誤請自行負責。

4. Please register by filling out the eForm. Participants are responsible for ensuring the accuracy of their information provided in the registration eForm. Incomplete or inaccurate registration data may result in certification errors. Participants bear responsibility for any errors on certificates due to incorrect information submitted.

五、開放報名日期：即日起到5/8額滿(50名)為止。超過50人第51人也可以參加但就沒有便當囉！

5. Registration Period: From now through May 4 (number of participants limited to 50). Above the limit, participants may still join the training, but NO lunch box will be provided to them.

六、急救訓練研習當天需參加實際操作考試合格後才有證照才有4小時證照。

6. Certificate will only be issued upon the successful completion of the hands-on practical exam during the workshop.

七、凡要上課者都要報名，因為證照上需要個資。

7. All participants must register for the course, as personal information (provided on the registration form) is required for the certificate.

八、如果要請公假，請於eForm電子表單中勾選，公假名單將以eForm電子表單填寫的為主。

8. If you need to apply for official leave, please select the appropriate option in the eForm. The list of approved official leaves will be based on the information submitted through the eForm.

To apply for official leave, please check the appropriate box on the eForm. The final list for leave approval will be based SOLELY on the data submitted via the eForm.
