【學務處衛生保健組】台北校區急救訓練研習報名至11/20截止-【Campus Health Services Section】 Taipei Campus First Aid Training Workshop Registration open through Nov 20

一、活動名稱：急救訓練研習(有證照)。

1. Activity Name: First Aid Training Workshop (certification included )

二、活動時間：115年11月26日（星期四）10:00-14:00

2. Date &  Time: 10:00-14:00, November 26, 2026 (Thursday)

三、活動地點：逸仙堂

3. Venue: Sun, Yat-Sen Auditorium

四、活動報名請填寫此eForm電子表單，資料須填寫完整且正確，否則證照資料錯誤請自行負責。

4. Please register by filling out the eForm. Participants are responsible for ensuring the accuracy of their information provided in the registration eForm. Incomplete or inaccurate registration data may result in certification errors. Participants bear responsibility for any errors on certificates due to incorrect information submitted.

五、開放報名日期：即日起到11/20額滿(50名)為止。

5. Registration Period: From now through November 20 (number of participants limited to 50).

六、急救訓練研習當天需參加實際操作，考試合格後才可領證照，且須全程參與到14:00才可領餐盒；修通識課-醫學與健康的同學可中午12:00離開，須簽退才有2小時證照(無餐盒)。

6. Participants must attend hands-on training and pass the examination to receive certification. Full attendance until 14:00 is required and a boxed lunch will be provided upon completion. Students enrolled in the General Education course Medicine and Health may leave at 12:00 PM. However, they must sign before departure to be eligible for the 2-hour certificate. No boxed lunch available for them.

七、修通識課-醫學與健康(原教室上課地點D102)的同學上完課12:00下課就要離開的同學，不要2小時證照”請勿報名”。

7. Students taking the General Education course Medicine and Health (original classroom D 102) who plan to leave after class at 14:00 and do not need the 2-hour certificate, please do not register.

*要兩個小時證照才要報名，因為證照上需要個資。

* Registration is required only for participants applying for the two-hour certification, their personal information is needed for certificate issuing.

八、若要請公假，請於eForm電子表單中勾選，公假名單將以eForm電子表單填寫的為主。

8. If you need to apply for official leave, please select the appropriate option in the eForm. The list of approved official leaves will be based on the information submitted through the eForm. To apply for official leave, please check the appropriate box on the eForm. The final list for leave approval will be based SOLELY on the data submitted via the eForm.
