【高教深耕計畫】115年度計畫申請-證照獎勵(一般生）（即日起至9/10截止）

各位師長 您好：

高教深耕計畫開放申請「115年度證照獎勵計畫」，懇請有意替學生申請獎勵之師長參閱以下計畫說明後，於eform填寫申請資料。
若師長對計畫申請內容、方式有任何疑問，懇請與高教深耕-怡虹 聯繫（分機2541）。

【計畫說明】：

★本表單僅限校內師長、職員填寫，欲申請之學生請通知師長、職員，由其代為申請。

1. 申請資格：凡本校學生於在學期間考取專業證照且申請時仍具本校學籍者（不包含陸生、海青班、博士生、在職專班），即符合本辦法之申請條件。
2. 獎勵證照：獎勵證照類別，以政府機構舉辦之公務人員考試、專門職業及技術人員高等或普通考試、專業技術人員技師類證照考試、全國技術士技能檢定、相關專業職業類檢定考試或專業職業類機構（如學會、協會、公會及法人機構等）所舉辦之專業技能檢定，取得相關證照且該證照符合教育部「教育部大學校院校務資料庫」採計，並經系所申請核准列入獎勵證照類（級）別表者（網址：https://tmap.mcu.edu.tw/），得申請獎勵。
3. 獎勵次數：同一申請人之同一項證照以申請一次證照獎勵為限；同一證照已獲得本校其他獎補助者，不得再重覆提出申請。
4. 獎勵金額：每一證照獎勵金額依課程地圖專業證照之一到四級考照費用八成為原則。其實際核給之金額、獲補助案件數及經費之增減，得視當年度經費總額與整體計畫發展，依實際情形調整之。
5. 認列期間：本次獎勵認列期間自115年4月15日起，至115年9月10日止，非屬該認列期間內者不予受理。
6. 申請資料：請將證照考取清冊、證照影本 PDF檔合併為1份檔案上傳，以作為審核資料。
7. 申請期間：自即日起至115年9月10日 中午12：00止。
8. 經費核銷：請務必於115年11月20日前 完成經費核銷。
9. 結案報告：請於經費核銷後7天內將結案報告電子檔上傳至moodle高教深耕計畫專區，同時應將獲證資料上傳至校庫。結案報告須包含證照考取清冊、證照影本。格式詳參以下網址：證照結案報告。

2026 Student Incentives for Obtaining Professional Certifications (Phase II)

The Higher Education Sprout Project Office is now accepting applications for the 2026 Student Incentives for Obtaining Professional Certifications (Phase II).

Faculty members and staff who wish to apply for certification incentives on behalf of students are kindly requested to review the following information and complete the application through this eForm.

1. Eligibility for Application

This eForm may only be completed and submitted by faculty members and staff. Students who wish to apply must contact a faculty member or staff member to submit the application on their behalf.

Eligible Students:
Ming Chuan University students who obtain a professional certification during their period of enrollment and maintain active student status at the time of application.

The following students are excluded:

• Students from mainland China
• Students enrolled in the Overseas Youth Vocational Training School (OYVTS)
• Doctoral students
• Students in in-service programs

2. Eligible Certifications

Eligible certifications include those obtained through government-sponsored examinations, specialized vocational and technical examinations, national skill certification examinations, or professional qualifications issued by recognized professional organizations.

The certification must be recognized by the Ministry of Education, listed in the University/College Database, and approved by the applicant's academic department as eligible for this incentive program.

3. Incentive Amount

The incentive generally covers up to 80% of the certification examination fee for Level 1 to Level 4 certifications listed in the University's Course Map.

Students with economically disadvantaged status may be eligible for a full subsidy. For details, please contact the Student Affairs Office (Ext. 2504 / 3904).

The actual subsidy amount, number of approved applications, and available funding may be adjusted according to the annual budget and overall implementation of the program.

4. Incentive Limitations

• The same applicant may apply for the incentive for the same certification only once.
• A certification that has previously received other funding or subsidies from Ming Chuan University may not be submitted again for this incentive.

5. Recognition Period

April 15, 2026 – September 10, 2026

Only certifications obtained within this period are eligible for this incentive.

6. Required Application Materials

Please upload one PDF file containing:

1. Certification list
2. Copies of relevant certification documents

Please combine all required documents into a single PDF file before uploading.

7. Application Deadline

Thursday, September 10, 2026, at 12:00 p.m.

Please ensure that the application is submitted before the deadline.

8. Reimbursement Deadline

November 20, 2026

All reimbursement procedures must be completed by this date.

9. Final Report

Within seven days after completing the reimbursement process, please:

• Upload the electronic version of the Final Report to the Higher Education Sprout Project section on Moodle.
• Upload the relevant certification materials to the University Database.

The Final Report must include the certification list and copies of the certifications.
