非常感謝你這學期每週勤奮不懈的努力，在前程預備結算各位的服務積分前，
請協助於12/13（五）下午5點以前完成這份期末回饋表，完成後可加總分10分哦！
其中第6~9項，請寫出具體資料(譬如：曾協助哪位同學／參與哪場活動），
經本處老師確認後，方予以加分。
最後，別忘了12/17（二）下午3點以前留意你的email信箱，
你將會收到一份【期末導師覆核單】，
請自行印出後交給導師覆核與簽名，
並於12/24（二）下午4點以前至前程規劃處（A棟2樓，富邦ATM樓上，近D105）

Dear Peer Support Coordinators on Taipei Campus,
Thank you so, so much for all the consistent effort you've put in as a Peer Support Coordinator for your class.
As we are about to calculate your service credits so that we can decide how many merits you will be awarded by MCU, we kindly ask that you help finish this feedback form *by noon, Friday, May 17.* You will get 10 extra credits once you finish this survey! Also, please don't forget to provide specific information as requested from questions 6 to 9 to help us confirm the services you have provided for your peers.
Lastly, don't forget to check your email by 3pm, Tuesday, Dec 17. We will send out a copy of your final report of services (as a peer support coordinator) for you and your class advisor to review. After you make sure that everything seems accurate, please print it out and ask your class advisor to review and sign. We kindly ask that you bring it back to Mr. Lin at the Career Planning and Counseling Division(on 2Fl. of Building A, near D105) by 4pm, Tuesday, Dec 24, to complete this entire task.
