        這份「學生心理健康關懷量表」可以幫助你了解自己的身心狀態，預估自我適應大學生活的情況，做為規劃大學生活的參考。

       並協助學校諮商輔導中心了解你的身心健康狀況，針對個人需求給予適當之關懷並提供你可以運用的資源。

　　此份測驗結果只有本校諮商輔導中心的輔導老師於必要時才可調閱，並遵守保密原則及相關倫理。

        若導師或其它人士申請調閱時，需經過你本人之同意。因此需要請你在下方勾選， 是否同意測驗結果讓導師知情。

Dear student, 

    Welcome to Ming Chuan University. This “Attentive Inventory of Mental Health” is an assessment which helps you better understand your mental health status as well as how you have adjusted to college life. The assessment result can also be served as a reference for MCU Counseling Center to understand your physical and mental health status. This allows us to further provide support and assistance for you based on your needs. 

    The assessment result is confidential and will only be evaluated by licensed mental health professionals from MCU Counseling Center when necessary. If your class advisor requests to view the result, no information will be disclosed to them without your consent. Therefore, we would like to confirm your willingness to disclose the assessment result to your class advisor.
