【服務學習時數】大四生、延修生服務學習時數彈性措施報名 Flexible/Remedial Measures for Service-Learning Hours Requirements before Graduation due to COVID-19 Epidemic

說明：

1.本表單僅供「大四畢業生」、「延修生」未完成服務學習時數者報名，其他年級請勿填寫。

2.將批次將報名者加入Moodle課程，後續請同學將製作完成的資料蒐集報告檔案上傳至Moodle作業區。

Explanation：

1.       For 4^th-year graduating students and extended study students who have not yet completed Service-Learning hours, the Career Planning and Counseling Division provides flexible measures.

2.       Those who want to adopt these flexible measures should register via e-Form first, and the applicants will be added to a Moodle course in batches. Afterwards, please upload the completed Information Collection Report files to Moodle.
