本表僅供申請共同英文課程必修換班填寫 This form is only for applying for a class section change for MCU Core Required English course(s).

113學年度第1學期原班必修換班、輔系、雙主修選課，請同學於2024/08/12(一)至08/19(一)於eForm上填寫申請。
2024-25 Academic Year 1st Semester Change of Class Section for Required Courses, Course Selection for Minors, and Double Majors, please fill out online e-Form from Monday, August 12 to Monday, August 19, 2024.

填寫此表需檢附：
1.完整「自我畢業審查」系統畫面，含表頭個人資料(參考圖)
2.個人課表
Please attach the following:
1. A complete screen shot of the Graduation Self-Check from your Student Information System which includes your personal information listed in the header
2. Course schedule

隨班重補修課程需於加退選時依照英語中心公告申請。
The retake courses need to be applied for according to the announcement from the English Language Center during the add/drop period.

教務處彙整學生申請資料後將轉請英語教學中心審核，經確認核可後再進行課程加退選作業，處理結果請於2024/09/05(四)16:00起自行上網查詢個人課表，確認結果。
The Academic Affairs Division will collect all applications and then submit them to English Language Center for further evaluation. Upon approval of the applications, the adding and/or dropping of courses will proceed. Please check the results that will be posted on your course schedule in the Student Information System starting at 16:00 on Thursday, September 5, 2024.
