因嚴重特殊傳染性肺炎疫情持續擴大，學校為瞭解目前居家隔離、檢疫、自主健康管理、14日內有出國旅遊史、發燒之學生確切人數，避免防疫缺口。請同學配合中央流行疫情指揮中心及教育部之政策，為降低傳染風險敬請協助問卷填答，共同維護校園健康。請同學正確填寫並將本調查表回傳，俾利衛保組依同學狀況提供健康關懷與專業諮詢服務，若有相關疑問或防疫諮詢或協助，請聯繫臺北衛保組、桃園醫務室(02-28824564分機2224.3172)

Since COVID-19 disease continues to expand, the university must have a clear understand of the actual number of students in home isolation, home quarantine, self-management of health, with overseas travel history within past 14 days, or having a fever. All students are to please follow the policies of Central Epidemic Command Center and Ministry of Education to complete this questionnaire with accurate information which can help Campus Health Services Section to follow different situations of students to provide related health care and professional consultation, lower infection risk on campus and maintain campus health. Health Care Reminders:

1. Wash your hands frequently with soap.
2. Wear surgical mask if you have a cough.
3. Reduce your time in public places or crowded areas. If necessary to be there, please wear a surgical mask.
4. If you feel sick, have fever or shortness of breath (upper respiratory tract illness), please see a doctor quickly; take leave from school and/or work; quarantine yourself stay at home and inform the Campus Health Services Section of Student Affairs Division.
5. Maintain a flow of fresh air indoors to lower the risk of contagion in classrooms or offices.
6. Increase daily vegetable and fruit consumption to improve immunity.
